Cyclone New Dealer Application
Click the image to the right to open, or right-click to download the application. The application is a fillable PDF form that can be completed in your web-browser or on your computer using free Adobe Reader software. You may also print and complete by hand.
IMPORTANT: The application must be signed. To sign digitally, you must use free Adobe Reader software and create a digital ID signature. If you do not sign digitally, please print and sign by hand.
When complete, save the application to your computer and upload it using the form to the right, email it to email@example.com, or print and mail or fax it to us.
MAC USERS: The features of this application will only work in Adobe Reader for Mac software. When you open the form in a web browser, the browser will use your default PDF file viewing application to fill out the form, which is often Mac Preview software. The form features will not work in Mac Preview. Please save the form and open it in Adobe Reader.
Which Pages To Fill Out
- New Dealers – Complete all three pages and sign pages 2 and 3.
- Existing Dealers who need to change payment information – Complete and sign the Credit Application section on Page 3.
- Existing Dealers who need to change contact or other shop informaton – Complete pages 1 and 2 and sign page 2.
More Detailed Instructions
1. Open The Application. The PDF file should open in a new browser tab. If you have an old browser, it may download to your computer instead.
2. Fill Out The Application If filling out electronically, all fillable fields are shaded in blue. When you click the digital signature box, Adobe Reader will lead you through the steps to create a digital signature. Please follow the steps and digitally sign. Remember, you can always print the form out and complete and sign by hand then send it back to us. If you’re filling it out in a browser window, we recommend saving the file to your computer as a backup. Click the “Save” button to do this.
Trouble filling it out? If for some reason you can’t fill it out, you may not have Adobe Reader software installed on you computer. You can download it for free here. If you have Reader installed but your browser just doesn’t want to cooperate, save the file to your computer (click the “Save” button or the picture of the disc in the toolbar at the top of the page to save), then open it from your computer to complete. You can also right-click the application image, then choose “Save as” to save the PDF to your computer.
3. Submit The Application
Once complete you may submit it to us in one of the following ways:
A) Save the file to your computer then upload it using the form above.
B) Save the file to your computer then email it to firstname.lastname@example.org.
C) Print the file and fax it to us at 503-226-0824.
D) Print the file and mail it to us at 2050 NW Vaughn St., Portland OR, 97209.
